Care and Case Manager Role Overview
This is a rewarding position that requires the ability to work with vulnerable individuals, providing support and guidance to help them achieve their goals.
* Assessing Holistic Needs: Evaluate the needs of clients and refer them to other support services as required.
* Social Care Practice: Demonstrate high standards of social care practice and adhere to tenancy laws.
* Rent Culture: Establish and maintain a positive rent culture with tenants, addressing obstacles and supporting strategies to empower tenants to adhere to rent and other areas of tenancy.
* Service User Support: Ensure the service responds effectively to the needs of service users in areas such as referral, induction, key working/case management, and property law and maintenance.
* Life and Home Making Skills: Develop life and home making skills essential for service users.
* Quality Standards: Maintain a consistently high standard of care for tenants and protect and promote their rights.
* Crisis Management: Handle difficult situations calmly and professionally, using crisis management techniques and following policy and procedures to ensure safety and relationships are maintained.
Key Responsibilities:
* Proactively identify and engage with tenants who require support, assessing how the service can assist, and prioritizing their support needs.
* Complete screening/initial assessments, develop and implement care/support plans, and conduct regular key working appointments as appropriate.
* Carry out risk assessments and risk management plans as necessary.
* Participate in multi-agency working through case management and case reviews as needed.
* Foster positive relationships with external agencies to maximize supports and resources available to clients.
Requirements:
* Possess relevant qualifications and experience in residential social care services, preferably in the homeless/supported housing sector.
* A full and clean driving license is also required.
