Job Title: Healthcare Assistant / Social Care Worker
GSW Solutions is recruiting for the role of Healthcare Assistant / Social Care Worker for our client in the Sligo area.
Purpose of the Role:
The Social Care Worker is expected to engage in daily activities and routines of the service, including direct support to service users, administrative work, and daily tasks. The SCW will work as part of a dedicated and innovative team within Residential Services, providing services to users in line with regulations and standards.
Main Responsibilities:
1. Support the day-to-day running of the service and deliver a person-centered approach.
2. Ensure that the Service User's personal plan is completed in accordance with regulations and standards, under the supervision of the line manager.
3. Proactively support service users in their daily lives and choices.
4. Maintain files to be inspection-ready at all times.
5. Engage with families, clinicians, and build community links.
6. Implement positive support programs.
7. Promote independence and community integration.
8. Stay updated on social care policies and sectoral changes related to disability services.
9. Motivate and support service users to engage in planned activities.
10. Lead key working in consultation with service users, encouraging engagement in meaningful activities.
11. Support the care of service users, including personal care and daily needs.
12. Educate yourself about each service user, listen actively, and use reflective practice to develop your skills and knowledge.
13. Follow, assess, and contribute to the development of strategies to meet ongoing support requirements.
14. Work as part of a team, supporting colleagues and attending regular meetings and supervisions.
15. Maintain confidentiality in line with GDPR.
16. Perform health and safety checks, risk assessments, and report accordingly.
17. Liaise with multidisciplinary teams, families, and relevant parties.
18. Observe and report health and safety issues.
19. Contribute to person-centered planning meetings and follow up on actions and outcomes.
20. Be flexible and willing to work shifts as needed.
21. Perform any other duties within the scope of the role to meet service requirements.
