The Role:
This is a full-time position reporting to the Community Service Manager through the Programmes Supervisor. The post holder will facilitate service users in their choices, developing programmes within the context of a person centred model of service delivery.
* This may include independent living programmes personal development activities occupational leisure activities within quality of life models
Your Responsibilities:
To be key workers for service users in order to facilitate Person Centred Planning processes.
* IDentify goals supports required by individual service users & maintain records as necessary
