Homecare Specialist
The role involves assessing patients' medical conditions, evaluating their needs, and determining the appropriate level of care and support required for home-based treatment.
Main Responsibilities:
* Conduct thorough assessments of Clients' medical conditions, functional abilities, and psychosocial needs to develop individualised care plans.
* Collaborate with healthcare professionals to gather and exchange information regarding Client care.
* Educate Clients and their families about their care plan and self-care techniques.
Key Qualifications:
* Bachelor's degree in a relevant healthcare field such as nursing or healthcare management.
* A valid nursing license or professional certification is required.
* A minimum of 2 years of clinical experience in a hospital or community healthcare setting.
