Role Overview
UPMC is building a dedicated Lifestyle Medicine service focused on prevention, behavioral change, and chronic disease risk reduction.
We are seeking a GP or consultant with a specialist interest in Lifestyle Medicine to lead and deliver evidence-based, patient-centred preventive care at our new clinic in Swords.
The successful candidate will champion lifestyle-based interventions across UPMC sites, work collaboratively within multidisciplinary teams and contribute to the strategic growth of UPMC's Lifestyle Medicine services in Ireland.
Key Responsibilities
Integrate Lifestyle Medicine into routine patient care and UPMC clinical pathways.
Promote preventive care and empower patients to manage cardiovascular, metabolic, and mental health risks.
Develop clinic-based and community-based programmes focused on behaviour change and sustainable lifestyle improvement.
Support audit, quality improvement, and outcome-driven service development.
Deliver comprehensive lifestyle assessments, including history, examinations, functional measures, and behavioural risk profiling.
Create personalized intervention plans covering physical activity, nutrition, stress management, sleep hygiene, smoking cessation, and weight optimization.
Lead programmes such as cardiac rehabilitation, weight management, smoking cessation, and stress reduction.
Order and interpret investigations (bloods, ECG, risk scores, functional tests) aligned with preventive care pathways.
Manage early-stage or lifestyle-responsive conditions such as hypertension, dyslipidemia, metabolic syndrome, obesity, and stress-related symptoms.
Coordinate care with specialists across cardiology, endocrinology, nutrition, psychology, physiotherapy, and sports & exercise medicine.
Participate in MDT meetings and contribute to pathway design and continuous refinement.
Deliver educational talks, workplace wellbeing engagements, and community outreach to promote lifestyle-based health solutions.
Maintain accurate clinical records, utilize UPMC digital systems for outcomes and data reporting, and support research, audit cycles, and quality improvement initiatives.
Ensure compliance with Irish guidelines, UPMC clinical governance, and data protection standards.
Qualifications & Experience
Registration: Irish Medical Council (IMC) – Specialist Division (General Practice) or Specialist Register.
Membership: Member of the Irish Society of Lifestyle Medicine or equivalent.
Minimum 3 years post-CSCST clinical experience with exposure to preventive medicine, chronic disease management, and health screening.
Demonstrated experience delivering or leading programmes related to physical activity, nutrition, stress, smoking cessation, or weight management.
Proven ability to work within integrated care teams.
Strong verbal, written, and IT capabilities for patient engagement, data management, and team coordination.
Experience engaging with external healthcare providers, community organisations, or insurers.
Core competencies: preventive health leadership, patient-centred care, programme development, education & advocacy, collaboration.
