About this Role
We are seeking a skilled professional to join our multidisciplinary team at a Community Nursing Unit. This role involves supporting elderly residents to maintain independence and quality of life through assessment, treatment planning, and rehabilitation.
Key Responsibilities
* Assess residents' functional abilities and create tailored care and rehabilitation plans
* Implement and review treatment programmes to promote independence and well-being
* Collaborate with healthcare professionals to deliver holistic care
* Educate and support residents, families, and caregivers on daily living strategies and equipment use
* Conduct outreach visits to provide additional support
Requirements
* Minimum 2 years' experience as an Occupational Therapist post-qualification
* CORU registration essential
* Full driving licence required for outreach work
