Lead Clinical Pharmacist in Oncology/Haematology
The successful candidate will be responsible for strategically leading and managing the delivery of clinical pharmacy services to Haematology/Oncology day wards and inpatient wards.
Main Responsibilities:
* To ensure patient care and safety are prioritized at all times.
* To lead, manage and develop pharmaceutical care and clinical pharmacy services.
* To achieve competence in using and administering the Oncology/Haematology clinical information system (ARIA).
* To review Systemic Anti-Cancer Therapy (SACT) protocols within the hospital.
* To manage safe and cost-effective entry of new anti-cancer medications onto the hospital formulary.
* To take a lead role in managing SACT medicine stock shortages.
* To assist in managing expanded access or compassionate use programs.
* To collaborate with the Multidisciplinary Team (MDT) to optimize SACT workflows.
* To contribute to developing clinical guidelines as part of the MDT.
* To identify and address deficiencies in medication management systems and processes.
Required Skills and Qualifications:
* A minimum of 4 years of post-registration hospital experience.
* Registration with the Pharmaceutical Society of Ireland.
* Significant, proven post-registration hospital practice experience.
* Broad clinical pharmacy experience and experience in line management.
* Experience working within an Aseptic Compounding Unit and/or haematology/oncology unit(s).
* Previous experience working within Cancer specialities at a senior level.
* Evidence of ongoing Continuing Professional Development and role-specific learning.
* Comprehensive knowledge and understanding of all relevant aspects of the National Cancer Control Programme (NCCP).
