The post of Clinical Nurse Manager 2 has a pivotal role in service planning, co-ordinating, and managing activity and resources within the integrated service. The main responsibilities are: quality assurance, resource management, effective communication, practice development, facilitating communication and professional/clinical leadership.
The role of the CCMT CNM2 is to:
* Assess patients with complex care needs.
* Attend MDT meetings as part of case management
* Scope out service solutions to barriers of discharge.
* Stratify risks.
* Identify and assist with the implementation of a plan for discharge via care co-ordination within the community services.
* Following up patients post discharge to the community for a defined time period and to Long Term Care Facilities carrying out assessment of needs at intervals as determined by the funding requirement
* Caseload management
Requirements:
* Experience with working with complex cases
* Knowledge of Acute and Community services.
