Job Title: Senior Occupational Therapist – Adult Primary Care
Location: Laois or Offaly
Contract Type: Full-time (35 hours per week, Monday-Friday)
Contract Duration: 5 month agency contract with potential for extension
Salary: HSE Senior Grade Occupational Therapist Pay Scales

Excel Recruitment are recruiting a Senior Occupational Therapist on behalf of a well-established Primary Care team based in Laois or Offaly. This is an outstanding opportunity for an adult trained Occupational Therapist to lead and deliver an adult focused Occupational Therapy service within a community setting.

You will join a well-established team of Healthcare professionals, where regular supervision, peer support, and innovation are at the core of service delivery.

Job Purpose

The successful candidate will deliver and lead evidence-based Occupational Therapy interventions for adults presenting with a broad range of physical, cognitive, and functional needs. Working as part of the Primary Care Team, you will enable service users to maximise independence, engage in meaningful daily activities, and participate fully within their community.

Clinical Responsibilities

1. Provide high-quality assessment, intervention, and review for adult clients, particularly older adults and those living with frailty.
2. Conduct home visits to assess functional ability, safety, environmental supports, and recommend appropriate equipment or modifications.
3. Deliver interventions focused on maximising independence, functional rehabilitation, falls prevention, cognitive assessment, and environmental adaptation.
4. Provide client-focused treatment in outpatient and community settings, developing tailored rehabilitation programmes and education.
5. Create, implement, and evaluate goal-focused care plans using best practice and clinical reasoning.
6. Monitor outcomes and adapt interventions in collaboration with clients, families, carers, and MDT colleagues.
7. Offer guidance and education to families and carers supporting independence at home.
8. Work collaboratively as part of a multidisciplinary Primary Care Team, including Physiotherapists, Public Health Nurses, GPs, Social Workers, SLTs, and other allied health professionals.
9. Liaise with hospitals, community organisations, voluntary agencies, and other stakeholders to ensure coordinated care.
10. Participate in integrated care and frailty pathway initiatives, conducting joint assessments where required.
11. Contribute to discharge planning, advocacy, and community reintegration.
12. Provide clinical leadership, supervision, and support to junior colleagues, therapy assistants, and students.

Requirements

13. CORU-registered Occupational Therapist (essential).
14. Recognised qualification in Occupational Therapy (CORU-approved).
15. Experienced Staff Grade OTs with strong adult experience will also be considered.
16. Experience in adult primary care, community OT, medical or rehabilitation settings.
17. Strong experience in functional assessment, frailty, rehabilitation, and working with older adults.
18. Demonstrated competence in home visits and community-based assessments.
19. Proven ability to independently manage a complex caseload.
20. Strong knowledge of equipment prescription, assistive technology, and environmental modifications.
21. Experience working within a multidisciplinary and interagency framework.
22. Excellent communication, clinical reasoning, and documentation skills.
