Growing together.As a Patient Care Coordinator (PCC), you'll serve as a key member of our primary care team, collaborating closely with nurse practitioners (NPs), physician assistants (PAs), and other interdisciplinary team members. This role supports multiple providers and plays a critical part in ensuring patients receive coordinated, high quality care.This is a field Based role, offering flexibility to support work life balance while meeting business and patient needs. PCCs work both from home and onsite in facilities. While travel to facilities is required, it is not expected every day, and team members may work from home several days per week when appropriate.Optum strives to assign work locations as close to home as possible, when feasible, to support work life balance—while still meeting operational needs.This position is full-time Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00am - 5:00pm. It may be necessary, given the business need, to work occasional overtime.We offer 2 months of paid training. The hours of the training will be based on schedule or will be discussed on your first day of employment.Primary Responsibilities:Support the interdisciplinary care team with overall member coordination under the direction of an RN or NP/PAPerform data entry and manage patient charts, including:New membersPost hospital visitsQuality initiativesLabs and medicationsObtain medical records following hospitalizations or physician appointmentsSchedule patient transportation and medical appointmentsServe as a flexible team member supporting both providers and patientsBuilding relationships within nursing facilities to supplement the programs missionWork Location, Travel & Mileage ReimbursementThis position requires travel to patient care facilities, though daily field visits are not required.
