Post specific related information: Qualifications required: A full QQI level 5 Health Care Qualification is essential (e.g. health, community, care sector) but a genuine passion for providing person centred support is key to being successful in the role of a Support Worker. Responsibilities
* Deliver support according to the person’s PCP and care plan.
* Place the needs of the person supported as the key focus in all activities.
* Engage in Programme Planning and Person Centred Plans.
* Provide support with an awareness of the Key Worker/Support Worker role and philosophy surrounding support to people with complex needs.
* Communicate effectively in fluent English and maintain excellent communication skills.
* Work on own initiative as required.
Qualifications and Abilities
* A full QQI level 5 Health Care Qualification is essential (e.g. health, community, care sector).
* Genuine passion for providing person centred support.
* Ability to work on own initiative.
* Awareness of Key Worker/Support Worker role and philosophy surrounding support to people with complex needs.
* Programme Planning and Person Centred Plans.
* Fluent English and excellent communication skills.
* Full, clean drivers licence essential.
