Clinical Nurse Manager, CNM – Nursing Unit, Dublin South.

Full Time Permanent Role

Salary, Dependent on Experience

Cpl Healthcare in partnership with our client seek a Clinical Nurse Manager – Patient Flow and Care Transition for Nursing Unit in Dublin 18.

These units are members of a large group, offering incredible support and infrastructure to new appointees. The group was founded in, operates a total of 32 nursing and care homes in Ireland.

Overview of the role:

To work as part of the Patient Flow team with the ADON, Patient Flow Manager. To ensure the safe and effective management of patient referrals to Mount Carmel Community Hospital from the acute hospitals. To facilitate admission and follow through regarding discharge planning. To act as case manager for patients as required.

Duties and responsibilities:

1. Receive patient referrals from acute hospitals; review referrals and liaise with Discharge Coordinators.
2. Ensure that there is a discharge plan and anticipated date of discharge for all patients referred to the unit.
3. Arrange for accepted referrals to be admitted to the unit.
4. Act as case manager for patients as required and as agreed by Patient Flow.
5. Adhere to all company policies and procedures.
6. Maintain clear, concise, and informative assessments and records within the hospital in accordance with legislation and company standards.
7. Knowledge and awareness of HIQA standards (Safer, Better Healthcare).
8. Ensure all admissions to the hospital are carried out correctly and all appropriate documentation completed.
9. Attend ward handovers daily and weekly MDT meetings.
10. Provide cover for the wards if required as necessary.
11. Liaise with doctors about admissions, prescriptions and ensure admission paperwork is ready each day.
12. Follow-up on patient's outpatient appointments if necessary.
13. Facilitate patient admissions/discharges.
14. Refer patients to Medical Social Worker as required.
15. Ensure effective communication with all referral sources in the hospitals and discharge agencies in the community.

Eligible applicants are invited to apply:

16. 3 years post-registration experience in a relevant field and experience in managing patient caseload.
17. Hold a recognised Managerial Qualification (minimum level 6 on the National Framework of Qualifications).
18. Registered on the NMBI live register.
19. Clinical experience working in older persons care or nursing home care.
20. Working experience of HIQA Standards and Health Act Regulations .
21. Fluent in English language (verbal and written) and be eligible to work in Ireland.
22. Strong communication and listening skills.
23. Ability to work well as part of a care team.
24. Caring and compassionate and be passionate about providing high quality person-centred care.
25. Have a basic knowledge of IT systems such as excel spreadsheets.
26. Have a good understanding of applications for home supports and NHSS/Fair deal.
