Clarecare’s headquarters are based in Ennis with local offices in Bushypark, Shannon, Kilrush, Killaloe and Ennistymon.Role and ResponsibilitiesJob DescriptionTitle: Clinical Nurse – Home Support Service – East ClareRole: Full-time, Permanent position overseeing clinical guidance to front-line staff, Home Support Area Managers and other service areas within Clarecare. Manage workload prioritisation and maintain attention to detail. Refer to full outline of position below.The Clinical Nurse is responsible for supporting best practice care to all service users in the designated area. They will provide clinical guidance to front-line staff and Home Support Area Managers to foster a clinical learning environment. They will promote a person-centred, community-focused model of care with consideration for carers. They will support staff through continuing professional development by keeping up to date with research and developments in Disabilities and Older Persons Services.JOB RELATIONS: The Clinical Nurse will work as part of the Clarecare team and report to the Home Support – Clinical Lead or designate. May be required to work outside normal hours. Flexibility and willingness to travel throughout Co. Clare is essential.HOURS OF WORK: Monday to Friday, 9:30 a.m. – 5:00 p.m. with 30 minutes paid break. Some evenings or weekends may be required and flexibility is essential.ANNUAL LEAVE: 26 days annual leave per full leave year (Jan–Dec), excluding Public Holiday entitlement.JOB LOCATION: Remote/Hybrid not applicable. On-site/community-based role. Office base: Clarecare’s office, Killaloe, Co. Clare. Travel within Co. Clare to Clarecare’s other Centre locations and to service users in the community is required. Clare.Provide safe, comprehensive nursing care in accordance with the Code of Professional Conduct (NMBI) and related guidelines.Develop and review care practice policies, procedures and guidelines; implement audit portfolio and ensure compliance obligations are met.Facilitate external audits (e.g., HSE/HIQA) and implement learnings.Promote continuing professional development and act as a clinical support resource for staff and management.Respond to care practice issues and incidents; participate in pilot projects and dissemination of training materials.Represent Clarecare at local/national fora and maintain professional standards of communication.Provide clinical guidance to front-line staff and Home Support Area Managers as appropriate; lead on practice development.Support compliance with legal requirements and Home Support standards as issued by HIQA.Motivate staff to deliver best practice and maintain high quality care standards.Develop and document quality and compliant care policies with the Clinical Lead and Compliance Service.Produce high-quality audit reports, statistics, and research outputs; investigate incidents and implement recommendations.Respect the rights, dignity and confidentiality of service users and carers/advocates.Contribute to clinical decisions within Clarecare Home Support Service.Communications: excellent verbal and written communication; liaise with staff, managers and external providers as needed.Build relationships internally and externally; represent the service when appropriate.Provide phone support and relay information to family carers; engage with other service providers, health professionals and referral sources.Accountability/Responsibility/Professionalism:Engage in continuing professional development and stay current with gerontology and disability services.Maintain high standards of behaviour and performance; ensure safeguards for Clarecare’s integrity.Plan, prioritise and manage time effectively; maintain professional boundaries and accurate client records.Flexibility to work irregular hours, including occasional weekends.Person Specification:Be registered or eligible for registration with the Nursing and Midwifery Board of Ireland (NMBI).Have at least 3 years post-registration experience, of which 2 years must be in Gerontology or Older Persons Services.Demonstrate evidence of continuing professional development.Have the requisite knowledge and ability to discharge the functions of the role.Annual Registration: Maintain live annual registration with NMBI and provide current certification annually.Other Essential Requirements:Positive fitness-to-work medical clearance and full health capability for duties.Positive Garda vetting disclosure through Clarecare.Two positive written references from current and most recent employer.Use of own car with full clean driving licence and adequate insurance to cover work use and indemnify Clarecare.Note: The foregoing outlines main duties and may not cover all tasks.
