Planner (Outreach Services)
Contract: Permanent 35 hours per week
Location : Co. Kildare
Senior Supervisor/Instructor Salary Scale DOE: (€40,521-€54,705)
The Role:
The Planner works as part of the Outreach team, planning with individuals who use Kare's Adult Services and has responsibility in supporting individuals who use the Outreach service to achieve their goals.
Duties & Responsibilities:
Ensure an Individual Support Plan is developed for each person supported in line with Kare's Policy Individualised Planning.
Support individuals to enhance their life by ensuring that goals and objectives outlined in their Individual Support Plan are achieved.
Complete a full Assessment of Need for all individuals you support in Outreach.
Support all individuals where appropriate with taking responsibility for all aspects of their health and wellbeing.
Promote autonomy for all service users with regards to all aspects of their finances.
Support each person where appropriate with regards to their personal and intimate care in accordance with KARE's Personal and Intimate Care Policy
Ensure that people who use the Outreach service are supported to take part in the Advocacy structure within Kare.
Ensure the concepts and values of the Supported Employment process are promoted.
Facilitate lifelong learning opportunities for people who use the service.
Requirements:
Minimum Qualification: QQI Level 5 Major Award in Intellectual Disability Practice or equivalent Level 5 Major Award or Social Care Degree
Possess a good understanding of Assisted Decision-Making Act and promote it when supporting individuals in Outreach.
3 years' experience working in a Local (Day) Service/Community House
A proven track record in planning with individuals
Excellent communication and interpersonal skills
Strong administration, organisational, logistical and IT skills
Eligibility to work in Ireland without sponsorship is essential
Full clean driving licence and access to your own vehicle is essential.
