This is a full-time role that involves working with individuals, families and friends who are concerned about cognitive health or living with dementia. We provide support throughout the entire dementia journey.

Main Responsibilities:

* Acting as a key point of contact on an outreach basis for individuals concerned about cognitive health or living with dementia.

* Offering practical assistance, advice, emotional support and accurate information.

* Directing individuals and families to appropriate services and helping them navigate the healthcare and social care systems effectively.

Required Skills and Qualifications:
">
* A primary degree or equivalent in a relevant healthcare or social care discipline.
">
* A strong understanding of dementia and its impacts.
">
* Practical experience working with individuals with dementia.
">
* Familiarity with the community care sector and a commitment to professional development.
">
* Awareness of legal frameworks related to vulnerable adults (e.g., Enduring Power of Attorney, Ward of Court).
">
* Strong communication, interpersonal and leadership skills.
">
* A client-focused approach.
">
* A full, clean driving license, as travel is a key part of the role.
