Job Title: Relief Programmes Facilitator

As a Relief Programmes Facilitator, you will play a key role in promoting independent living and community integration. You will work closely with service users to develop person-centred programmes that meet their unique needs and aspirations.
Key Responsibilities:
* Develop and deliver person-centred programmes to meet the needs of service users as identified through Person Centred Planning
* Maintain accurate records of programme development, implementation, and evaluation
* Support individuals in accessing community resources and activities to meet identified needs
You will be responsible for observing all rules and regulations related to our services. This includes adhering to administrative policies, procedures, rules relating employment law equality acts including general data protection regulation (GDPR)
