Job Title: Community Care Specialist
We are seeking a skilled and compassionate individual to join our team as a Community Care Specialist. In this role, you will be responsible for providing high-quality nursing care to patients in their homes.
About the Role:
The successful candidate will have experience in providing clinical services to patients, including cannulation and phlebotomy skills. They will also possess excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
Key Responsibilities:
* To provide high-quality nursing care to patients in their homes
* To maintain accurate electronic patient records
* To deliver reports promptly after each visit
Required Skills and Qualifications:
The ideal candidate will have demonstrated IV therapy experience, including care of central venous access devices. They will also possess a high degree of self-motivation, evidence of personal and professional development, effective administrative skills, good time management skills, computer literacy (Microsoft Office preferred), excellent communication skills (including presentation skills) and ability to build rapport quickly with new contacts.
