Introduction to the Model
The OFD project aims to develop effective and sustainable emergency accommodation and services for families and single individuals referred by the relevant local authority. The project worker will assess individual and family needs to establish appropriate services and supports, including schools, parenting, and childcare. The main goal is to ensure a coordinated response to access and exit suitable housing, healthcare, and social support to end homelessness and sustain long-term housing. Some casework will be allocated under the Housing Plus programme to support long-term tenants in crisis and ensure tenancy sustainment. The Good Shepherd Centre collaborates with Mental Health Service Area 5 and partners with Kilkenny, Tipperary, Laois, and Carlow County Councils.
Main Roles and Responsibilities
1. Work within the framework of the overall objectives and policies of the Kilkenny Voluntary Housing Association.
2. Conduct assessments and support planning with clients, focusing on recovery and life skills to promote tenancy sustainment.
3. Support tenants in accessing training, education, and employment opportunities.
4. Work as part of a multidisciplinary team and maintain a caseload of clients, providing ongoing support until cases are handed over to appropriate services.
5. Accompany and support clients with appointments to other services, as appropriate.
6. Maintain up-to-date casework records, daily logs, and assessments, including inputting information into the Focus Ireland database and PASS.
7. Collaborate with other teams across mental health, addiction, Tusla, schools, and community services, including statutory and voluntary agencies.
8. Adhere to service models and best practice standards.
9. Participate in supervision and training as required.
10. Supervise students or volunteers as directed.
11. Work a flexible roster over 7 days and evenings to meet service objectives.
12. Implement policies and procedures, and contribute to their development.
13. Undertake additional duties as assigned by the team leader or manager.
14. Ensure health, safety, and welfare risks are managed and reported.
Additional Responsibilities
Engage with service users to facilitate housing and recovery goals based on recovery principles of hope, control, and opportunity, using a strengths-based approach. Conduct needs assessments to inform care plans, support navigation of local authority processes, monitor housing options, and assist with viewings and welfare applications. Support service users in developing life skills, participating in community activities, and sustaining tenancies. Communicate with support services, facilitate peer support, and advocate as needed. Manage referrals to promote tenancy sustainability and independent living, ensuring safety, health, and wellbeing.
Essential Criteria
* Two years of post-qualification experience in a related field.
* Full clean driving license and access to a car.
* Level 7 or above in social care.
* Good verbal and written communication skills.
