Job Description

The Team Leader plays a pivotal role in ensuring the smooth transition of individuals into our service.

This involves designing and coordinating individual support programs that cater to their unique needs and abilities.

* Safely transitioning individuals into the service
* Liaising with families and multi-disciplinary teams
* Developing and implementing support programs
* Ongoing assessment and review
* Ensuring medication procedures are followed
* Providing leadership, supervision, and management
* Participating in learning and development opportunities
* Maintaining accurate records


Required Skills and Qualifications
