Applications are invited from suitably qualified candidates for the following post: Quality and Patient Safety Manager Grade VIII Each candidate must meet the following criteria: 1. An academic award in Healthcare Management, or Quality in Healthcare at level 7 (or higher) on the Quality & Qualifications Ireland (QQI) framework or equivalent OR A professional qualification in a health related area AND 2. Significant experience of working in the health services in a post that has involved quality and risk management, incident management and investigations as relevant to this role. 3. Significant experience of working at a senior level collaborating with multiple external and internal stakeholders in leading quality and safety improvement 4. Significant experience of leading on the implementation of corporate policies, procedures and guidelines in the areas of quality, risk and patient safety. 5. Experience of leading change in a complex organisation. 6. Have the requisite knowledge and ability (including a high standard of suitability and management ability) for the proper discharge of the duties of the office. The post holder will Lead and champion all issues related to patient safety, patient experience and incident reporting. Provide clear leadership and direction on the implementation of the hospitals accountability and governance structure for quality and safety. Co-ordinate and monitor the status of recommendations from investigations, reports, audits. Co-ordinate the collection of information linked to objectives and key performance indicators and ensure processes are in place to evaluate performance. Support the RVEEH in the implementation of quality and patient safety standards incorporating continuous improvement, risk identification and control Ensure that a culture of quality improvement and accreditation is maintained and promoted in the RVEEH. Lead communications for clinical risk / complaints queries from the Office of the Ombudsman, Office of the CEO, State Claims Agency, HIQA etc Support the HMG in identifying strategic priorities for quality, safety and risk improvement and in monitoring compliance with relevant national standards and regulatory requirements. Key responsibilities Operational Management Act as a focal point for all internal and external Quality & Patient Safety (QPS) Reporting. Lead the strengthening and integration of the hospitals quality improvement, assurance and risk management structures to support effective governance, oversight and continuous improvement. Monitor, evaluate and report on the implementation of the plan across the hospital in accordance with defined and agreed reporting structures. Review and make recommendations on all significant quality and safety concerns indicating whether they have been correctly identified and are being appropriately managed. Work with the Clinical Director, Nurse Management, HSCPs and other professionals to reduce clinical incidents and make evidential improvements to the quality of care and the safety of care of patients through hospital initiatives. Establish appropriate communication and management structures and processes for safety incident management within the hospital. Co-ordinate the collection, measurement and presentation of data required for monitoring quality and patient safety indicators. Keep abreast of quality, safety and risk management issues and co-ordinate implementation of actions arising from developments in national and international practice. Act as Access Officer for RVEEH Quality Improvement, Assurance and Patient Experience Provide strategic oversight for the development and strengthening of the hospitals quality, patient safety and assurance framework, aligned with National Standards, the HSE Service Plan, Clinical Care Programmes, national policy priorities and HIQA regulatory requirements and JCI. Promote and support patient engagement and the effective use of patient experience to inform service improvement, organisational learning and quality assurance. Support HMG and multidisciplinary services in identifying, implementing and monitoring quality improvement priorities across the hospital. Promote and support the consistent application of quality improvement methodologies and assurance processes across clinical and operational services. Ensure effective systems are in place for the collection, analysis and reporting of quality and patient safety data to support performance monitoring, compliance and continuous improvement. Lead and oversee the hospitals accreditation, audit and quality assurance activities, including HIQA standards, regulatory compliance and associated action plans. Coordinate the collection of information linked to objectives and key performance indicators and ensure processes are in place to evaluate performance of relevant areas Provide ongoing monitoring of relevant recommendations and provide reports to relevant committees as required Incident / Complaint Management Systems Ensure effective governance and management of adverse incidents, complaints, claims and investigations, identifying trends, hospital learning and opportunities for quality and patient safety improvement. Liaise with external agencies including the States Claims agency, coroners, legal representatives and other regulatory bodies, ensuing timely collation and submission of relevant information. Provide timely reports, analysis and presentations to the HMG, Quality & Patient Safety Executive and any other relevant committees on risk, claims, incidents, investigations and compliance matters. Co-ordinate and maintain an effective incident, claims & complaints management system in the hospital, facilitating the development of relevant policies and procedures. Ensure mandatory reporting of incidents onto the National Incident Management System and onward to relevant stakeholders within mandated timeframes. Ensure compliments & complaints received by the RVEEH are responded to and comply with existing national frameworks and mandatory requirements. Review and maintain online reporting systems e.g. Qpulse, as effective incident, systems in the hospital. Participate in incident reviews and where required and/or appropriate, lead and support system analysis investigations. Facilitate & support family meetings as required. Ensure a process of communication and escalation of issues identified is in place for Clinical Risk & Patient Feedback & Complaints Ensure quality tracking and auditing processes are in place and monitored. Promote a quality & risk management culture in project delivery through the development of appropriate quality & risk strategies and systems, supported by best practice tools, methodologies and performance measures Accreditation & Compliance Have a significant working knowledge of the Health Service Executives Incident Management System, Patient Safety Strategy, Health Information and Quality Authority (HIQA) Standards, Joint Commission International (JCI) Standards, Health Service Executive (HSE) Standards and the Quality Framework and associated local/national protocols for implementing and maintaining these standards as appropriate to the role. Work with members of the HMG, Heads of Departments and individual departments across the hospital to develop and support the implementation of plans to achieve and maintain quality & patient safety standards Advise the HMG and Quality and Patient Safety Executive (QPSE) on compliance issues relating to HIQA, HSE, national / international standards adopted by the hospital. Ensure outcomes of reviews are appropriately communicated and actioned Data Management Be a key contact for the HMG for data relating to Quality, Risk and Patient Safety information; for example including but not limited to Council reports (internal), Quality and Patient Safety Executive Reports and Hospital Patient Safety Indicator Reports In conjunction with the Risk Health & Safety Manager ensure that appropriate data management systems are in place to support the hospital patient safety programme. These systems include Incident Reporting, Risk Registers, Safety Alerts, NQAIS Clinical, and National Audit of Hospital Mortality (NAHM) Analyse data to monitor performance and improvement against quality standards, audit recommendations and patient safety goals Coordinate and validate patient safety indicator reports and performance reports as required, for any relevant meetings Ensure appropriate benchmarking of relevant data, initiating corrective action for any adverse findings in the data management process.
