Key Responsibilities:
1. To develop and deliver person-centered programs that cater to the unique needs of service users as identified through Person-Centered Planning. Maintain accurate records.
2. Act as a key worker for service users facilitating Person-Centered Planning process while identifying individual goals and necessary supports maintaining relevant records thereof.
