RGN required for Respiratory Integrated Care team in Co Donegal
This is a post within the Respiratory Integrated Care team undertaking a Slaintecare Virtual Ward Project.
The role requires cover from Monday to Friday, 8am-4pm, excluding weekends and bank holidays.
It entails working within a community environment as part of a Multidisciplinary team. The staff member would be based in Letterkenny but need to travel within Donegal due to the RIC service's operation across four network locations.
Preferable Requirements if possible:
* Two years' post-registration general medical nursing experience.
* Experience in respiratory chronic disease nursing care is desirable.
The role involves working within the Respiratory Integrated Care Team (RICT).
Responsibilities include:
* Assessing, planning, implementing, and evaluating nursing and respiratory care.
* Monitoring and managing patient symptoms.
* Participating in analysis of remote monitoring data to identify potential issues and provide early intervention.
* Providing individualized patient-centered care virtually and face-to-face when necessary.
* Carrying out home visits to assess patient needs, monitor progress, and liaise with the RIC team.
* Educating patients and their families about their respiratory condition, treatment plans, medication management, and lifestyle changes.
* Offering open-access telephone support to patients and families.
* Maintaining links and communicating effectively with community services through the multidisciplinary team, patient, and family to ensure continuity of patient care.
* Referring to other services as necessary and signposting to other support services.
* Discussing discharge planning and ongoing care.
