Care Coordinator Role
We are seeking an experienced professional to oversee the coordination of care assistants schedules on a daily basis, address operational issues, and ensure clients have a permanent care team assigned. This is an onsite position at our office.
Key Responsibilities:
* Coordinating care assistants schedules and ensuring seamless workflow
* Addressing operational issues related to customer service calls in a timely manner
* Ensuring all clients have a permanent care team assigned for their benefit
* Dealing with scheduling queries and responding promptly to minimize delays
* Providing emergency cover until the next working day
* Informing clients of any schedule changes
* Assigning schedules two weeks in advance to maintain consistency
* Verifying weekly allocated hours to prevent discrepancies
* Liaising with the Care Manager on start dates for new carers to facilitate a smooth onboarding process
* Reporting completed tasks by the end of the working day
* Ensuring all shifts are covered two weeks in advance to maintain a balanced workload
* Conducting regular reporting to monitor progress and identify areas for improvement
Our ideal candidate has a minimum one year experience in a similar role and strong organizational skills.
