Discharge Coordinator – Operations
Role Purpose: The discharge coordinator delivers high-quality administrative support within a clinical area, ensuring effective coordination of patient discharges in line with organisational standards and continuous quality improvement.
Key Areas of Responsibility
Act as a liaison with the multi-disciplinary team to conduct accurate assessments, review home set-up, and coordinate community funding/resources.
Provide patients, relatives, and carers with discharge planning information, timescales, and support in collaboration with Nurses/CNMs and the MDT.
Coordinate with care and nursing homes to ensure timely assessments and documentation for discharge.
Arrange transport booking for patient discharge and coordinate with the nursing team.
Attend daily bed management meetings to discuss and coordinate discharge planning for new admissions.
Ensure appropriate discharge plans, including home‑care packages, public‑health nurse referrals, and community services, are in place.
Maintain accurate records for service evaluation and development.
Plan patient transition from an acute hospital to an appropriate setting and liaise with ward staff for delivery and education.
Collaborate with other departments to ensure safe and efficient patient discharge.
Promote a patient‑centred culture of openness, responsiveness, dignity, respect, and choice.
Communicate with the Senior Medical Social Worker regarding discharge issues.
Communication Skills
Communicate with care agencies about patient discharge needs.
Maintain professional communication with colleagues and healthcare teams, avoiding conflict and reducing anxiety.
Attend and contribute to relevant meetings; accept constructive advice.
Report incidents immediately that may adversely affect patients, colleagues, or the clinic.
General Expectations
Support the clinic’s philosophy, objectives, and goals.
Adhere to lines of authority and maintain strict confidentiality of patient information.
Demonstrate flexibility and assist in all areas of the hospital or clinic as required.
Participate in the development of quality care, assist new personnel orientation, and pursue personal knowledge development.
Maintain a professional appearance, dress code, hygiene, and infection‑control policies; be punctual.
Qualifications
Minimum 4 years of clinical experience.
Relevant registration with the governing body.
Prior experience in a hospital setting desirable.
Valid work permission for the Republic of Ireland (or eligibility for a work permit).
