Job Description
Processes medical records for admissions and re-admissions. Compiles data from records for periodic statistical reports. Assists in completion of charts by working with physicians. Logs charts in and out of the computer system.
Job Responsibilities
1. Performs medical record clerical functions including:
o Locates and retrieves records as needed for patient care, chart completion, and studies.
o Assists physicians with the chart completion process.
o Receives, separates, sorts, and verifies receipt of dictated reports and refers questions to transcription. Follows up on outstanding reports, files originals, and mails copies to physicians.
o Ensures receipt of charts into the department utilizing the ADT Report.
o Verifies and corrects, as necessary, patient name and unit number in the SMS system. Records and processes canceled admissions.
o Facilitates record completion including faxing, mailing, or delivering reports and records.
o Files charts and loose materials into the permanent filing area and incomplete area. Purges charts as needed.
o Determines where the chart goes next and forwards to the proper location.
o Places records into the proper files or creates a new folder for discharged patients.
2. Prepares deficient medical records data. Maintains deficient medical records and physician visits in the chart deficiency system.
3. Submits current physician suspension list and incomplete operative reports daily.
4. Performs related duties as required.

Job Qualifications
* High School Diploma or equivalent, required.
* Minimum of one (1) year clerical experience, required. Medical Records experience preferred.
* Computer skills, required.
