Excel Recruitment are recruiting an experienced Occupational Therapist on behalf of a well-established Primary Care Team based in Ballyfermot Primary Care Centre. This is an excellent opportunity for an adult-trained Occupational Therapist to lead and deliver a high-quality, adult-focused Occupational Therapy service within a community setting.
Overview:
The successful candidate will deliver evidence based Occupational Therapy interventions for adults presenting with a wide range of physical, cognitive, and functional needs.
You will have a comprehensive clinical induction, access to some HSE training as required by the role (determined by OT manager).
You will have a clinical supervision with an Occupational Therapy manager. And peer support of a small, but experienced, OT Department.
Clinical Responsibilities:
1. Provide high-quality assessment, intervention, and review for adult clients, particularly older adults and individuals living with frailty.
2. Conduct home visits to assess functional ability, safety, environmental supports, and recommend appropriate equipment or adaptations.
3. Deliver interventions focused on functional rehabilitation, falls prevention, cognitive assessment, and environmental modification.
4. Provide client-centred interventions in outpatient and community settings, developing tailored rehabilitation programmes and education plans.
5. Develop, implement, and evaluate goal-focused care plans using best practice and sound clinical reasoning.
6. Monitor outcomes and adapt interventions in collaboration with clients, families, carers, and MDT colleagues.
7. Provide education and guidance to families and carers to support independence at home.
8. Work collaboratively within a multidisciplinary Primary Care Team, including Physiotherapists, Public Health Nurses, GPs, Social Workers, Speech and Language Therapists, and other allied health professionals.
9. Liaise with hospitals, community services, voluntary agencies, and relevant stakeholders to ensure coordinated and integrated care.
10. Participate in integrated care pathways and frailty initiatives, including joint assessments where required.
11. Contribute to discharge planning, advocacy, and community reintegration.
12. Provide clinical leadership, supervision, and support to junior staff, therapy assistants, and students.
Requirements
13. CORU registration as an Occupational Therapist (essential).
14. Recognised CORU-approved qualification in Occupational Therapy.
15. Preferably have some adult primary care, adult medical, adult disability, seating, complex cases, neurological or palliative care experience.
16. Access to a car (almost all work is taking place in service users homes in / around the Ballyfermot / Palmerston / Inchicore / Cherry Orchard area).
17. Have an enthusiastic attitude and be keen to take on what is a challenging work environment -meaning lower than average staffing levels, and lengthy waiting times.
18. Strong knowledge of equipment prescription, assistive technology, and environmental modifications.
19. Experience working within a multidisciplinary and interagency framework.
20. Excellent communication, clinical reasoning, and documentation skills.
