Contract: 2 Years Fixed-Term, Full-time
The purpose of this role is to work as part of the Aftercare Team to provide care and support for young people exiting the care system who are most at risk of homelessness.
What you will do:
* Work within the framework of the overall aims and objectives of Focus Ireland.
* Develop suitable care plans to prepare and support residents towards independent living, using the Housing First for Youth Model.
* Support young people holistically, focusing on practical self-care, health, budgeting, education, training, and domestic skills. Promote confidence and self-esteem among young people leaving care.
* Advocate for residents regarding housing, social work, social welfare, education, training, and legal rights.
* Make referrals to relevant outside agencies and liaise with them.
* Monitor health, safety, and welfare risks, reporting concerns to your line manager or Health & Safety Representative.
* Attend team, divisional, and agency meetings as required.
* Maintain accurate case records and statistics.
* Report to external stakeholders when necessary.
* Keep the team leader informed about issues arising from individual cases.
* Participate in supervision sessions regularly.
* Engage in relevant training, development courses, and project evaluation.
* Implement project and organizational policies, adhering to customer service standards.
* Assist in implementing Focus Ireland's settlement policies and strategies.
* Collaborate with other Focus Ireland projects, particularly Focus Housing.
* Undertake additional duties as assigned by the Project Leader, Section Manager, or Divisional Head in consultation.
What you will need:
* Extensive relevant experience working with young people in care or marginalized youth.
* Third-level qualification in social care or a related field.
* Experience in key-working clients or individual case management.
* Experience managing challenging behavior.
* Full clean driver's license.
* Good understanding of issues affecting young people leaving care and in aftercare.
* Flexibility.
* Understanding of Child Protection policies.
