This role offers a stimulating environment to contribute to the assessment, treatment, and discharge planning of patients across various acute inpatient wards, including medical, surgical, orthopaedics, and rehabilitation.
About the Position:
You will work collaboratively with a supportive team, receiving senior guidance, and participate in early mobilisation, safe discharge, and improved patient outcomes across the board.
Key Responsibilities:
* Conduct comprehensive initial and ongoing assessments of patients using relevant outcome measures and clinical reasoning.
* Develop and implement individualised treatment and rehabilitation plans based on patient-specific goals.
* Deliver evidence-based physiotherapy interventions including: Mobilisation and rehabilitation post-surgery, Neurological rehabilitation, functional, and strength-based rehab for frailty, deconditioning, or long-term conditions.
* Manage a diverse caseload independently across acute medical, surgical, orthopaedics, neurology, ICU, or care of the elderly.
* Prioritise patients effectively based on clinical urgency and ward demands.
* Participate in rapid discharge pathways, early supported discharge, and weekend planning.
* Actively engage in daily ward rounds, MDT case reviews, and discharge planning meetings.
* Collaborate with consultants, nurses, occupational therapists, social workers, and discharge coordinators to ensure safe and timely discharges.
* Communicate effectively with patients and families about goals, progress, and post-discharge care.
