About the Job
The role of a Service Planner involves collaborating with individuals who require support services. This individual will work closely with the Outreach team to provide personalized planning and support.
Key Responsibilities:
1. Develop comprehensive Support Plans tailored to each person's needs.
2. Support individuals in achieving their goals and enhancing their overall well-being.
3. Conduct thorough Assessments of Need for all supported individuals.
4. Promote autonomy and facilitate lifelong learning opportunities.
Required Skills and Qualifications:
* QQI Level 5 Major Award in Intellectual Disability Practice or equivalent degree.
* Minimum 3 years of experience working in a Local Service/Community House environment.
* Excellent communication and interpersonal skills.
* Eligibility to work in Ireland without sponsorship requirements.
