Role summary
The Maternal Medicine Clinic (MMC) Pharmacist will work as part of a specialist multidisciplinary team (MDT) to optimise medication use in women with complex medical conditions before, during and after pregnancy. Reporting to the Pharmacist Executive Manager (PEM), the post holder will provide expert clinical pharmacy input to maternal medicine services, ensuring safe, evidence-based and patient-centred pharmacotherapy across the continuum of care including pre-conception, antenatal, intrapartum and postnatal periods.
The role involves clinical assessment of medication regimens in high-risk obstetric patients, provision of specialist medicines information relating to pregnancy and lactation, and development of individualised pharmaceutical care plans in collaboration with obstetricians, physicians, midwives and other healthcare professionals.
The MMC Pharmacist will play a key role in medication reconciliation, teratology risk assessment, patient counselling, and planning for delivery and postnatal management. The post holder will also contribute to the development of clinical guidelines, patient and healthcare professional information resources, and education programmes.
The role will involve collaboration at local and national level, including engagement with the Maternal & Newborn Clinical Management System (MN-CMS) and cross-site initiatives to standardise and enhance medicines optimisation in maternal medicine services.
Reporting relationship
Reporting relationship is to the Pharmacist Executive Manager at The National Maternity Hospital. The post holder will also collaborate closely with senior obstetricians leading the MMC.

Provide specialist clinical pharmacy services to the Maternal Medicine Clinic MDT.
Conduct comprehensive medication reviews for obstetric patients with co-morbidities and complex pharmacotherapy.
Undertake medication reconciliation at key transition points including booking and admission.
Develop and document individualised pharmaceutical care plans.
Provide pre-conception counselling regarding medication use in pregnancy and medication optimisation.
Support medication planning for labour, delivery and the postnatal period, including breastfeeding considerations.
Perform teratology risk assessments using evidence-based resources.
Provide patient-centred counselling on medication use in pregnancy and lactation.
Identify and manage medication-related problems.
Act as a specialist resource for medicines information in pregnancy and lactation.
Develop and maintain clinical guidelines, protocols and patient information resources.
Deliver education and training to healthcare professionals.
Participate in audit, research and quality improvement initiatives.
Support optimisation of medication-related functionality within MN-CMS electronic healthcare record.
Collaborate with MDT members and across maternity sites.
Maintain a clinical pharmacy workload and participate in departmental activities.
Ensure compliance with pharmacy legislation and professional standards.
