About the Role
As a Community Support Worker, you will play a vital role in enabling service users to live independent lives within their home and community.
You will be responsible for developing and delivering person-centred plans that cater to the unique needs and aspirations of each individual.
Key Responsibilities:
* Enable Service Users to live independent lives in their home and community.
* Support the development and delivery of person-centred plans.
* Establish partnerships with local communities to foster active citizenship in line with service user goals and wishes.
* Transport service users to access the community.
* Be responsible for Key Worker duties i.e. weekly administration, advocacy, liaison with families and develop and implement person-centred plans.
Requirements
* Hold a relevant QQI qualification - minimum Level 5 in Healthcare or relevant.
* Experience working with service users in the disability sector.
* Be over the age of 18.
* Have a full clean manual driving license with access to own vehicle for work purposes.
* Have an excellent command of spoken and written English.
* Have previous experience working with individuals with disabilities or in relevant sectors.
