Job Overview
We are seeking a highly skilled Social Care Specialist to join our team.
* The successful candidate will work closely with the Service Manager to ensure the smooth day-to-day operation of the service.
This is an exciting opportunity for a dedicated individual who is passionate about making a positive difference in the lives of others.
You will be responsible for processing referrals and conducting assessments, developing and implementing care and support plans, engaging with tenants and supporting them with housing sustainment plans, and addressing maintenance issues.
This role requires a collaborative approach with the Local Authority and a thorough knowledge of principles of Social Care/Community Work, health and safety legislation, data protection, report writing, funding structures for homeless services, and Case Management, Key working & Holistic Needs Assessment.
To succeed in this role, you will need excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
