Role Overview: A Client Care Coordinator ensures clients receive high-quality care by coordinating services, managing schedules, and communicating effectively with clients, care teams, and healthcare professionals. They maintain accurate records, handle inquiries, and assist with administrative tasks.
Key Responsibilities:
* Scheduling and Coordination: Managing client care schedules, ensuring appropriate coverage, and adjusting schedules as needed.
* Communication: Acting as a liaison between clients, their families, care team members, and healthcare professionals.
* Client Support: Addressing client inquiries, concerns, and requests, providing support and guidance.
* Record Keeping: Maintaining accurate and up-to-date client records, documentation, and care plans.
* Quality Assurance: Monitoring client care delivery to ensure high standards are maintained.
* Administrative Tasks: Assisting with data entry, filing, report generation, and other administrative duties.
Skills and Experience:
* Excellent communication and interpersonal skills, both verbal and written.
* Strong organisational skills to manage multiple tasks and maintain accurate records.
* Problem-solving abilities to assess situations and implement solutions.
* Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and other relevant software.
* Prior healthcare experience or working with individuals in need of care is preferred.
