Job Summary
We are seeking a compassionate, organised, and experienced Care Manager to oversee and coordinate high‑quality homecare services for a diverse client base, including older adults, individuals with physical and intellectual disabilities, and children with complex care needs. The Care Manager will assess client needs, develop person‑centred care plans, support care staff, and ensure services are delivered safely, effectively, and in compliance with all regulatory and safeguarding standards. Experience supporting individuals with intellectual disabilities and behaviours that challenge is essential.
Key Responsibilities
Conduct initial assessments and regular reviews for clients receiving homecare services
Develop, implement, and monitor personalised, person‑centred care plans tailored to each client’s needs
Coordinate care packages for elderly clients, individuals with physical and intellectual disabilities, and children with complex care needs
Ensure care plans are outcome‑focused, strengths‑based, and regularly updated
Support clients with intellectual disabilities, autism, sensory needs, physical disabilities, and behaviours that challenge
Coordinate and oversee complex care supports including nutritional support, respiratory care, epilepsy management, mobility, and behavioural support plans
Liaise with families, multidisciplinary teams, schools, therapists, and healthcare professionals to ensure continuity of care
Conduct risk assessments and ensure appropriate safeguarding and behavioural support measures are in place
Promote independence, wellbeing, social inclusion, and community participation for clients
Ensure care staff and nurses are trained, supported, and competent in delivering specialist and complex care supports
Maintain accurate client records, care documentation, incident reports, and risk assessments
Participate in audits, quality improvement initiatives, and regulatory inspections
Ensure confidentiality and GDPR compliance
Requirements & Qualifications
Nursing qualification essential – RGN or RIND
Previous experience in care coordination, care management, or a supervisory role within homecare, disability, or community care services
Experience supporting elderly clients, individuals with disabilities, behaviours that challenge, and children with complex care needs
Strong knowledge of safeguarding, risk management, complaint management, and person‑centred care planning
Excellent communication, organisational, and leadership skills
General qualification in “Trainer the Trainer” or interest in pursuing the same
Full driving licence and access to a vehicle
