Details of Service:
The Dublin North City and West and Dublin North County Social Inclusion – Addiction Services is primary responsibility is to improve health outcomes for persons with addiction issues, homeless refugees, asylum seeker and Traveller and Roma communities. There is an identified need to develop an Integrated Care Pathway in collaboration with HSE, Hospitals and Community and Voluntary addiction services. Addiction service management and development will be a key component of this role to ensure a seamless continuum of care is in place.

Social inclusion is within Primary Care and works across a range of statutory services in partnership with the community and voluntary sectors to address health inequalities and improve access to health services for vulnerable and excluded groups. The improvement of health outcomes for socially excluded groups in society is a key priority for this Division. The social inclusion officer is a new post in line with Social inclusion's key priority, with a targeted approach to identifying and supporting the needs of social inclusion's identified groups outlined below. These posts will work under the Community Health Network Structure and be based in a network area to support the network Manager and Allied Health Professionals with care planning for marginalised groups. The Liaison Officer will be a part of Social Inclusion Team but work in North East Inner City (NEIC) Network. The purpose of the role will be to link and support people from vulnerable groups to access appointments and treatment in primary care, chronic disease hubs and hospitals.
