Medical Social Worker
for a reputed hospital in Dublin, Ireland.
Permanent Full Time
Responsibilities
Conduct psychosocial assessments for patients and families.
Identify emotional, social, financial, and environmental needs affecting patient care.
Develop personalised care and support plans.
Provide crisis intervention when required.
Arrange community supports such as home care and rehabilitation services.
Offer counselling and emotional support to patients and families.
Support end-of-life care discussions and bereavement needs.
Advocate for patients' rights and assist with accessing benefits and social supports.
Work closely with multidisciplinary teams to deliver holistic patient care.
Identify and report safeguarding or protection concerns for vulnerable individuals.
Link patients with appropriate community resources and support organisations.
Maintain accurate documentation and prepare reports as needed.
Requirements
Must be registered in the Social Workers Register maintained by the CORU Social Workers Registration Board and maintain active annual registration
Minimum 1 year post-qualification experience required
Recent acute medical experience and/or experience working within a multidisciplinary team (MDT) in an acute healthcare setting
Knowledge of relevant legislation and policies essential
