Responsibilities
Conduct assessments to identify client needs and develop tailored care plans.
Provide counseling and support to individuals and families dealing with various challenges.
Collaborate with multidisciplinary teams, including healthcare providers, to ensure comprehensive care.
Advocate for clients' rights and access to necessary services and resources.
Maintain accurate and confidential client records.
Offer crisis intervention and emergency support as needed.
Facilitate group sessions and workshops on relevant social issues.
Requirements
Bachelor's degree in Social Work or equivalent
Professional registration with CORU
Proven experience in primary care or similar healthcare setting.
Strong communication and interpersonal skills.
Ability to work collaboratively within a multidisciplinary team.
Excellent problem-solving and crisis management abilities.
Knowledge of local community resources and services.
Empathy, patience, and a non-judgmental approach.
Strong organizational skills and attention to detail.
Right to work in Ireland without restriction.
All candidates will be required to undergo Garda Vetting and International Police Clearance if applicable.
