In this role, you will provide medical support to our clients while ensuring a safe environment for them.

Responsibilities
Participate in planning and coordinating total patient care in conjunction with the RN and physician's plan of care, following the nursing care plan for each patient.
Accurately report and record the patient's condition and care, including signs and symptoms indicative of change.
Prepare clinical/progress notes and summaries.
Assist patients with activities of daily living and teach appropriate self-care techniques.
Provide and maintain a safe environment for patients.
Assist physicians and Registered Nurses in complex nursing situations.
Follow universal precautions during all patient care activities.
Follow emergency procedures in case of catastrophic events, incidents, or significant changes in patient condition.
Qualifications
1 year of experience as an LPN in a home care setting.
Current state license as an LPN.
Graduate of an accredited practical nursing program.
Proof of current CPR certification.
Ability to work a flexible schedule.
Excellent interpersonal skills.
Valid driver's license and reliable transportation.
Willingness to travel locally and work flexible hours.
