Health Inclusion Worker, Housing First, Mid WestLocation: Limerick & ClareContract: Full Time, 1 year fixed termThe Housing First Service delivers a 'Housing First' approach to individuals experiencing homelessness across Limerick and Clare. This programme focuses on providing wraparound health supports within tenancies to promote stability and independence. It addresses complex health needs, including general health, mental health, and addiction, to support long-term housing success.What you will do:Work within the objectives, policies, and procedures of the Housing First Service, ensuring compliance with Health & Safety standards.Provide a safe, welcoming environment for customers who are homeless or at risk, respecting choice and self-determination.Undertake assessments and develop support plans using the Care and Case Management model.Accompany and support customers at appointments with health, social, and community services.Assist customers in integrating into their local community and accessing clinical, medical, and other supports.Support customers in developing life skills and exploring education, training, and employment opportunities.Maintain an appropriate caseload and keep accurate casework records, daily logs, and assessments, ensuring timely data entry into Focus Ireland systems (including PASS).Provide advice, information, and guidance to prevent homelessness or respond to out-of-home situations.Work as part of a multi-disciplinary team, attending meetings, handovers, training, and supervision sessions as required.Implement existing policies and contribute to the development of new ones, ensuring best practice standards.Be vigilant to health, safety, and welfare risks, reporting concerns promptly to the line manager.Participate in relevant training and development courses and undertake other reasonable duties as assigned.Ensure customers receive ongoing health care for general health, mental health, and substance use needs.Provide tailored one-to-one support in mental health and addiction.Facilitate access to local health services and liaise with key professionals such as Public Health Nurses.Involve customers in project operations to the greatest practical extent and monitor progress as requested.What you will need:A Third Level qualification at degree level in the area of social care i.e. youth work, social studies, addiction, psychology etc and/or be a CORU registered Social Care Worker .Two years' experience of working with single homeless people, substance users, people experiencing poor general and/or mental health and/or other socially marginalised groups.Experience of working with customers at risk. Experience of team working. Report writing abilities.Experience of working and liaising with other agencies, including both voluntary & statutory Full Driver's License with access to a car.Excellent communication, interpersonal and report writing skills.(Desirable)Knowledge and understanding of Housing First and issues affecting people with complex general & mental health and addiction needs.(Desirable)Ability to plan and implement programmes.(Desirable)Focus Ireland is an equal opportunities employer#FI2
