Job Title: Clinical Nurse Specialist - Palliative Care Reporting To: Director of Nursing/Assistant Director of Nursing Job Summary : To ensure maximum support and continuity of care for patients, and their family, in this specialised area of Palliative Care etc. and to develop the service in line with needs of patients. Main Duties: In collaboration with other members of the multi-disciplinary team, assess the needs of the patients, families and other carers and provide specific advice/support as appropriate. This will include advising and supporting the multi-disciplinary teams to enable them to provide high quality care, for example giving advice on pain and symptom control, care planning, social and psychological care, communication and resources as appropriate. Participate in the provision of information to patients and their carers regarding their disease, treatment and services available. Identify the specific education needs of individual patients and carers and facilitate meeting those needs. Work in an advisor and complementary role to the primary care team in providing specialist palliative care to those referred to the service who are assessed as requiring this level of care. Provide pain and symptom control, advice and support to patients with palliative care needs. Undertake an initial assessment of referred patients to ascertain their particular care needs. Liaise with all other relevant healthcare professionals involved in their patients' care so as to determine the patients needs. Demonstrate clinical excellence and a consistently high standard of nursing practice in the field of palliative care nursing. Develops and maintains effective liaison with community professions, support services and other hospices and palliative care teams. Facilitate the provision of pastoral care and bereavement support to families and other carers. Efficiently utilize the resources required to meet service objectives. Identify the training and education needs of the multi-disciplinary team together with other senior staff to develop specific skills related to palliative care. To work collaboratively with relevant educational staff to provide teaching programmes to meet the needs of staff within our catchment area. Develop a culture of evidence based practice within the service and, in consultation with managers, to identify and encourage the use of research-based practice with regard to palliative care. Develop mechanisms for auditing care within the service. In conjunction with senior medical and nursing staff, participate in the development of multi-disciplinary audit and implement changing practice as a result. Participate in or support, where appropriate, nursing and/or collaborative clinical research in cancer and palliative nursing. To provide a service of specialist palliative care nursing to patients and families within their own home.
