About the Role
We are seeking a highly skilled and compassionate senior physiotherapist to join our multidisciplinary team. As a key member of our primary care community team, you will play a vital role in assessing, treating, and supporting patients in their own homes or in our community-based settings.
* Conduct comprehensive physiotherapy assessments and develop personalized treatment plans that address each patient's unique needs and goals.
* Work collaboratively with our occupational therapy colleagues, other healthcare professionals, and support staff to deliver patient-centered care and achieve optimal outcomes.
* Foster strong relationships with patients, families, and caregivers to ensure seamless transitions and continuity of care.
* Promote early hospital discharge and prevent unnecessary readmissions through proactive rehabilitation and reablement interventions in the community.
Requirements
* Bachelor's degree in physiotherapy from an accredited institution.
* CORU registration as a physiotherapist.
* Minimum three years of post-graduate experience, with at least one year at senior level (preferably in a community or rehabilitation setting).
