The post holder will work directly with individuals and their families in their own homes, assessing needs, planning supports, and promoting independence, dignity, and quality of life.This role involves close collaboration with families, carers, and multidisciplinary professionals and plays a key role in early intervention, ongoing monitoring, and care planning.Key ResponsibilitiesConduct regular home visits to older adultsCarry out assessments covering cognitive function, daily living skills, emotional wellbeing, safety, and social supportsIdentify risks within the home environment and recommend practical adaptations or supportsContribute to care planning and review outcomes on an ongoing basisDeliver compassionate, respectful, and individualised dementia support in the client's homePromote independence, choice, dignity, and autonomySupport emotional wellbeing, orientation, and cognitive stimulationDevelop and implement tailored support plans and activity programmes based on assessment outcomesProvide guidance on meaningful activities that can be continued at home by clients and familiesAdjust interventions in line with progression of dementiaEssential RequirementsRelevant qualification in Social Care, Nursing, Psychology, Gerontology, Occupational Therapy Assistant, or related disciplineDemonstrated experience supporting older adults living with dementiaExperience or confidence in conducting assessments in client homesStrong report writing and documentation skillsExcellent communication, empathy, and organisational skillsFull driving licence and access to own vehicleEligibility to work in IrelandBenefitsCompetitive SalaryPaid MileageSupportive EnvironmentHybrid model (office and home based)
