€57,936 - €62,834 per annum dependent on experience
Location:
The base for this role will be at MQI's Head Office in Dublin 8 and involves consistent travelling to all of our services across the country.
Role:
To serve as the MQI central point for client movement through treatment and recovery services.
This role is designed to ensure that no matter where a person comes from, be it from custody, the community, detox or outreach, they are seamlessly transitioned into the most appropriate service.
This role includes advocating on behalf of people and dismantling barriers to create frictionless pathways between prisons, residential treatment, day programmes and aftercare services so that people receive the highest level of care required to achieve and sustain meaningful recovery.
Key Responsibilities
Strategic Case Management & Referral
Manage a complex caseload of individuals who have been assessed, navigating the most appropriate pathways through prison and community settings.
Negotiate varying service criteria to ensure seamless access to detox, residential treatment, therapeutic day programmes and aftercare.
Ensure all clinical documentation (initial assessments, comprehensive needs assessments, medical reports, risk assessments etc.) are recorded and transferred accurately to receiving services to maintain continuity of care.
Advocacy & Barrier Dismantling
Act as a strong advocate for clients to ensure they receive the highest standard of care regardless of their background.
Identify systemic barriers to access (e.g., funding, lack of ID, medical cards, housing stability) and work proactively with stakeholders to resolve them.
Support clients in navigating the transition, ensuring emotional and practical supports are in place during the high-risk period of release/transfer/progression.
Stakeholder Engagement & Partnership
Develop and maintain professional relationships with IPS, Probation, HSE, Counsellors, Key Workers and other supporting services and stakeholders.
Liaise with managers of residential and day services to negotiate placements and stay updated on availability, criteria and waiting times.
Ensure effective handovers at all transition points.
Represent the organisation at multi-agency & multi-disciplinary meetings.
Clinical Governance & Planning
Collaborate with prison-based and community-based staff teams to develop and monitor multi-disciplinary, person-centred care plans.
Track the outcomes of referrals to ensure service effectiveness and client retention through to aftercare.
Person Specification
Essential Criteria
3rd Level Qualification in Addiction or related field.
Minimum of three years' post-qualification experience working directly with individuals accessing treatment services.
Expertise in conducting comprehensive needs assessments, as well as developing, monitoring, and reviewing multi-disciplinary care plans.
Track record of interagency collaboration and the ability to navigate complex services alongside multiple stakeholders.
Experience advocating for people from marginalised groups.
Experience in Risk Assessment and the implementation of safety plans for people with complex needs.
Knowledge and understanding of the National Drug strategy and the National Drug Rehabilitation Framework (NDRF).
Training and experience in specific modalities like Community Reinforcement Approach (CRA), Motivational Interviewing (MI) and Cognitive Behavioural Therapy (CBT).
Proficiency in Group Facilitation, with the ability to lead or support therapeutic or educational sessions effectively.
A commitment to, and practical application of, Trauma-Informed Practice (TIP) principles across all aspects of service delivery.
Strong interpersonal skills with the ability to manage professional boundaries.
Proficient in report writing, accurate data collection, and maintaining records in line with GDPR and clinical governance standards.
High level of proficiency in Microsoft Office 365 (Teams, Excel, Word) for remote collaboration and data reporting.
Desirable Criteria
Experience in the formal or informal management of a multi-disciplinary team, including supervision, performance monitoring, and professional development.
Existing connections with local community resources, housing agencies, and health services.
Experience supporting individuals within the Irish Prison Service (IPS) or providing resettlement support to those in the post-release/probation phase.
An understanding of the specific challenges faced by individuals transitioning from custody to the community, from detox to rehabilitation programmes and aftercare.
Experience using case management software (e.g., ECASS, Salesforce, or HRB/NDTRS reporting systems)
Possession of a full, clean driving licence and access to own vehicle for work-related travel.
