Job Overview
We are seeking a dedicated professional to fill the role of Social Care Worker in public health.
This is a community-based position supporting vulnerable individuals from groups experiencing marginalisation, focusing on people with complex social problems and unmet healthcare needs.
The main duties include:
* Promote the welfare and support the needs of service users.
* Liaise with family, general services, and other agencies on behalf of the service user.
* Support engagement/attendance at medical and clinical appointments.
* Implement effective conflict management practices.
* Attend team meetings and report to the manager on matters affecting the delivery of service.
* Be accountable for any expenses incurred during the course of duty.
* Compile and submit regular reports to the manager on the progress/development of service user care plans.
* Document and report any incidents, however minor, that may occur during duty hours.
* Stay up-to-date with current legislation and professional care knowledge.
* Contribute to ongoing professional development through training and supervision.
* Perform any additional duties assigned from time to time.
Requirements
A recognised Social Care qualification or equivalent experience.
CORU registration or proof of application for CORU registration.
Previous experience in a similar role.
Strong administration and ICT skills.
Excellent communication and interpersonal skills.
