Purpose of the Post
To provide a comprehensive pharmacy service in Letterkenny University Hospital in co-operation with existing pharmacy staff.
Principal Duties And Responsibilities
The post holder will support the principle that care of the patient comes first at all times and will approach their work with the flexibility and enthusiasm necessary to make this principle a reality for every patient to the greatest possible degree
Maintain awareness of the primacy of the patient in relation to all hospital activities.
Performance management systems are part of the role and you will be required to participate in the Group's performance management programme
The principal duties and responsibilities of the Pharmacist, Senior include:
Provision of a comprehensive pharmaceutical service including:
Assist in managing the efficient running of the Pharmacy dispensary.
Supervise the supply of drugs, pharmaceuticals and other related items, as may be required.
Participate in the development and expansion of ward pharmacy / clinical pharmacy services.
Assist the Pharmacy Executive Manager, their deputy and Pharmacy staff in the performance of his/her duties and responsibilities, as required.
Undertake such other duties as appropriate to the office as may be assigned by line management from time to time.
Oversee the operation of the pharmacy dispensary.
Oversee the operation of the Pharmacy Aseptic Unit, including the compounding and external purchase of chemotherapy, monoclonal antibodies and associated medications.
Participate in various hospital committees and forums as assigned by the Pharmacy Executive Manager.
Liaise with local mental health professionals and manage pharmacy involvement in the out-patient dispensing of Clozapine and other psychiatric medications to patients in the Donegal area.
Liaise with the Pharmacy Executive Manager and Pharmacy staff on matters concerning departmental procedures, safety, security and complaints concerning the service.
Ensure that the purchase, storage and supply of all items is operated on the most economical lines, consistent with quality at pharmacy, ward and department levels and with reference to legal requirements, transit, security and conditions of chemical and physical stability.
Maintain records for purchasing, quality control, compounding and dispensing to the standards required.
Answer and record medicines information queries from staff and patients.
Provide advice, as required, to medical and nursing staff on the proper and economic use of drugs and medicines including participation in schemes for the dissemination of information in relation to drugs and drug usage designed to deliver optimal care economically.
Provide a patient focused clinical pharmacy service including prescription review, patient counselling, and medication reconciliation as necessary.
Ensure that the needs of patients and professional staff are being adequately met by the pharmaceutical service provided. This will involve providing help and advice to patients and staff on all aspects of therapy and collaborating with community pharmacists to ensure continuity of supply of medication where necessary.
Ensure the safe use of medicines by evaluating and managing risks associated with the use of medicines and by participating in the hospital's incident reporting process.
To participate in Antimicrobial Stewardship activities as relevant to the role.
Educational
Deliver training and education to medical, nursing and pharmacy staff through induction days, study days, journal club and other educational events.
Supervise and tutor pharmacy staff and students during work experience placements or initiation training in the department.
Participate in continuing education and in such activities consistent with the post and in accordance with PSI requirements. Attend such study days and continuing education courses as may be deemed necessary for the development of both the service and the individual.
General
Develop and implement policies, procedures and guidelines relating to medicines management.
Carry out general administrative and financial duties including recording keeping and drug use monitoring.
Participate in multidisciplinary working groups and committees as required, including hospital accreditation.
Supervise and manage any pharmacist or other staff assigned to him / her.
To assist and cooperate with the Pharmacy Executive Manager in the performance of his / her duties and responsibilities as required.
Perform such other duties appropriate to the office of Senior Pharmacist as may be assigned to him / her from time to time by the Pharmacy Executive Manager.
Conduct research and perform role related audits on the use of medicines and medicines management as per direction of the Pharmacy Executive Manager.
Health & Safety

Every employee must use safety equipment or clothing in a proper manner and for the purpose intended
Any employee who intentionally or recklessly misuses anything supplied in the interests of health and safety will be subject to disciplinary procedures.
Every employee must work in accordance with any health and safety procedures, instructions or training that has been given.
No employee may undertake any task for which they have not been authorised and for which they are not adequately trained.
Every employee is required to bring to the attention of a responsible person any perceived shortcoming in our safety arrangements or any defects in work equipment.
All employees are under a duty to familiarise themselves with the Risk Management/Fire, Health & Safety Policies.
All staff are responsible for identifying, assessing and reporting all risks and for contributing to the management and review of all risks.
Employees must attend fire lectures periodically and must observe fire orders.
In line with the Public Health (Tobacco) (Amendment) Act 2004, smoking within the Hospital Building is not permitted.
All accidents within the Department must be reported immediately.
Have a working knowledge of the Health Information and Quality Authority (HIQA) Standards as they apply to the role for example, Standards for Healthcare, National Standards for the Prevention and Control of Healthcare Associated Infections, Hygiene Standards etc and comply with associated HSE protocols for implementing and maintaining these standards as appropriate to the role.

KPI's
The identification and development of Key Performance Indicators (KPIs) which are congruent with the Hospital's service plan targets.
The development of Action Plans to address KPI targets.
Driving and promoting a Performance Management culture.
In conjunction with line manager assist in the development of a Performance Management system for your profession.
The management and delivery of KPIs as a routine and core business objective.

The management of Risk, Infection Control, Hygiene Services and Health & Safety is the responsibility of everyone and will be achieved within a progressive, honest and open environment.
The post holder must be familiar with the necessary education, training and support to enable them to meet this responsibility.

The post holder is responsible for ensuring that they become familiar with the requirements stated within the Risk Management Strategy and that they comply with the Group's Risk Management Incident/Near miss reporting Policies and Procedures.
The post holder is responsible for ensuring that they comply with hygiene services requirements in your area of responsibility. Hygiene Services incorporates environment and facilities, hand hygiene, catering, cleaning, the management of laundry, waste, sharps and equipment.
The post holder must foster and support a quality improvement culture through-out your area of responsibility in relation to hygiene services.
The post holders' responsibility for Quality & Risk Management, Hygiene Services and Health & Safety will be clarified to you in the induction process and by your line manager.
