The role of a Support Worker is to provide high-quality care and support to children with disabilities.

Key Responsibilities:

* Assist in the review of clinical reports from private providers, documenting outcomes and recommendations in a standardized template under the supervision of the Children's Disability Network Manager.
* Support the transfer or referral of clients to other services where appropriate, under the supervision of the Children's Disability Network Manager and clinical team.
* Contribute to the discharge of clients from the service, working closely with the Children's Disability Network Manager and clinical team.
* Collaborate on file reviews and prepare summaries of clinical notes, ensuring accuracy and adherence to local guidelines.
* Take an active role in the administration and day-to-day operation of the service, responding to emerging needs as they arise.
* Research and develop resources for universal family support and strategies, promoting best practices.
* Participate in team meetings and report on matters impacting service delivery to the Children's Disability Network Manager.
* Maintain exceptional documentation standards, adhering to local guidelines, confidentiality principles, Freedom of Information (FOI), and GDPR Acts.
* Contribute to the development and implementation of information sharing protocols and audit systems, ensuring seamless operations.
* Promote ongoing service development, aligning with good practice and HSE objectives.
* Foster professional standards in consent, confidentiality, ethics, and legislation, maintaining a safe and respectful work environment.
* OBSERVE all Health and Safety and Conduct at Work rules, utilizing equipment responsibly.
* Requirements: QQI Level 5 Award in Health Care Support/Social Care/Special Needs Assisting or equivalent; experience supporting children or individuals with disabilities; understanding of person-centred care and rights-based approaches.
