Due to continued growth, we are now seeking a motivated Care Manager to join our team.
This role will be based in our South Dublin office.

This is a permanent, full-time position, with core working hours of 9:00am - 5:00pm, Monday to Friday.
Flexibility is required, as occasional evening and weekend work may be necessary.
About the Role The Care Manager plays a key role in ensuring the delivery of high-quality, person-centred care to both private and HSE-funded clients.
You will manage client care from initial assessment through to ongoing service delivery, while supporting and guiding a team of caregivers to ensure excellent standards of care.
You will also be responsible for supporting business development, building relationships within the community, and contributing to the continued growth of the service.
Key Responsibilities: Carry out client consultations and comprehensive care assessments Develop and maintain person-centred care plans and ensure all documentation is completed Coordinate the smooth set-up and ongoing management of care packages Build and maintain strong relationships with clients, families, and caregivers Liaise with Public Health Nurses, the Quality Team, and other healthcare professionals Manage new client enquiries, including long-day and live-in care arrangements Accept and process new care packages and increases in hours Lead weekly planning to ensure all visits are scheduled and covered Maintain accurate records, including the HSE referral tracker Report and escalate any clinical or operational concerns to senior management Ensure caregiver supervisions are up to date and provide ongoing mentoring and support Support new carers through first-day supervisions, checks, inductions, and client introductions Support business development by building networks, identifying opportunities, and strengthening relationships with healthcare professionals and community partners Requirements: Previous experience in homecare or community-based care is highly desirable Full clean driver's licence Strong organisational and problem-solving skills Ability to work under pressure and use initiative Excellent communication and interpersonal skills A genuine passion for supporting older people in the community Desirable: Registered Nurse or qualification in Nursing or Social Care Management INDDUN
