Role Purpose:
This enhanced primary care post sits within a dynamic and diverse multi-professional team driving innovation in an enhanced primary care models based on the patient centered medical home.
This post is a new opportunity to apply your GP skillset in a diverse, dynamic multi professional team driving innovation in providing excellent integrated care for our VHI patients .
Straddling the existing care models of Vhi, Urgent Care, screening and speciality clinics already offered you will be involved in delivering enhanced primary care across all areas with a particular focus on growing and developing integrated primary care services within Vhi
Application of your Primary care skillset in a progressive, well-funded model of care which strives to provide the highest quality of excellent patient care.
You will join a multidisciplinary team of health professionals including a clinical lead with background in primary care, nurses and a range of AHSCPs in the delivery of effective and high quality primary care, encompassing medical screening to all patients
The role has scope for national delivery of care, however your primary base will be the Vhi 360 Centre in Balally/Cork/Galway, with a requirement to be flexible to work between these locations if required.
There will be opportunity for the successful post-holder to develop and maintain sub-speciality interests.
Key Working Relationships:
* Deliver and develop primary care services within the Vhi network and create an excellent model of care to provide the best services and care for your patients
* Apply your General Practice skills in a new dynamic and well organized system
* Engage with other primary care colleagues within the Vhi network and 360 Centre to provide enhanced ambulatory community care to address patient needs.
* Support an integrated care approach across Vhi H&W and related service including but not limited to urgent care and hospital at home
* services.
* Promote and develop disease prevention measures.
* Champion clinical excellence and patient-centric healthcare engaging voice of customer and capturing patient reported outcomes
* To participate in the delivery of effective clinical governance and to ensure that processes are in place which address clinical effectiveness and outcomes, clinical risk, clinical incidents, patient experience and complaints.
* Work with the relevant Quality, Safety & Risk (QSR) team members to ensure that the design and delivery of enhanced primary care services adhere to H&W QSR policies, procedures, protocols and guidelines (PPPGs) and external standards / statutory requirements (e.g. HIQA).
* To identify education requirements and opportunities and provide education to staff.
* Review and interpret results and ensure critical results are escalated appropriately.
* Complete medical reports for patients based on agreed protocols.
* Communicate to patients, provide relevant lifestyle advice and ensure patient understanding of their results.
* Participate in clinical audits, quality improvement and patient safety initiatives.
* Work as part of a team to ensure patients receive excellent clinical and customer-focused experience.
* Participate in clinical meetings and stay up-to-date with latest evidence based clinical practice and guidelines.
* Provide advice to other clinical colleagues when or as required as set out in SOPs.
* Maintain ongoing CME.
* Undertake any other clinical duties as may arise from time to time within the Vhi 360 clinics
* Participate in and support any accreditation programme through compliance with SOPs and clinical best practice, as laid out in the relevant standards.

Skills and Abilities
Technical:
* Experience of developing, applying and reviewing an evidence-based approach to decision-making
* Excellent communication skills, both verbal and written
* Demonstrate evidence of effective planning and organising skills and has the ability to manage multiple complex tasks.
Interpersonal:
* Operate with professional and personal integrity coupled with an overarching humility and proven ability to build strong working relationships.
* Demonstrate an interest in, and be an advocate for all aspects of patient care
* Demonstrates a match to values of Vhi health and wellbeing including commitment to care for patients by leading with heart/ courage to create a new future and together we do our best work
Qualification
* Hold specialist registration with the Irish Medial Council, in the division of General Practice
* Minimum 5 years relevant clinical experience (inclusive of GP Training).
Knowledge & Experience
Essential
* Experience working with multi-professional, multi-disciplinary teams
* Experience facilitating patient pathways from community to tertiary care
Desirable
* Postgraduate training in area of interest such as Women's Health, Paediatrics, Dermatology, DME .
* Advocate of value based care, experience of developing patient reported outcomes and delivering outcome based clinical practice. Quality Improvement interest
* Previous experience in Emergency Care and Ambulatory care
* Special interest in education / Innovation
