Overview Connolly Hospital Blanchardstown (CHB) Diabetes service based in Diabetes Day Centre provides specialist diabetes care for over 6000 patients with Type I and Type II diabetes. The CNM2 (CNS) in Diabetes provides self-management education and specialist support to inpatients, outpatients and by phone to all individuals with diabetes, within the Endocrine service within his/her scope of practice. The CNM2 (CNS) in Diabetes engages with a broad spectrum of individuals with diabetes, from adolescent through to elderly care. The Diabetes Service in CHB consists of specialist multidisciplinary input including Consultant Endocrinologist, CNS Diabetes/RANP, Dietician, NCHD's Podiatrist, Psychologist, and Research Nurse. The CNM2 / CNS will develop key competencies in this specialist role with the support and guidance of the endocrine consultants, ANP's/CNS staff members of the MDT in the DDC The CNM2 / CNS (Diabetes) will deliver effective quality assured patient centred care. The CNS has a pivotal role in co-ordinating and further developing the established service at CHB. The CNM2 (CNS) Diabetes will deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse Specialist Posts, 4 th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008 Post Specific Requirements: Has a post graduate qualification in Diabetes nursing or make a commitment to undertake a post graduate qualification in Diabetes care within 2 years of commencement in post.
