Family Support Specialist
The primary objective of this role is to engage with families and individuals facing complex needs, utilizing an Intensive Care and Case Management Model.
Key responsibilities include working collaboratively within an intensive family therapeutic support framework to assess needs, advocate on behalf of clients, provide tailored support, and facilitate referrals as necessary. The position requires adaptability in engaging families, establishing connections with diverse services, and contributing to case conferencing models.
To succeed in this position, candidates should possess a degree in Social Care or a related field, accompanied by at least 2 years of experience working with high-need families utilizing an intensive case management and family support model. Strong communication, computer, and report writing skills are essential for success in this role.
